Privacy Policy

Healthy Lumberjack operates in Canada and complies with the Personal Information Protection and Electronic Documents Act (PIPEDA), applicable provincial health privacy legislation such as Ontario's Personal Health Information Protection Act (PHIPA), and guidance from the College of Dietitians of Ontario and Dietitians of Canada.

1. Information We Collect

We collect personal information that you provide when booking an appointment or completing the intake form, including identity details (name, date of birth), contact information (email, phone, address), health history, nutrition goals, insurance eligibility, and any documents or images you upload. We also collect limited technical data such as device, browser, and interaction logs through our booking tools.

2. How We Use Your Information

We use your information to deliver virtual dietetic services, personalize care plans, coordinate benefits and billing, communicate about appointments, and improve our clinical programs. We may de-identify data for quality improvement, planning, or reporting trends that do not identify you.

3. Consent and Your Rights

By submitting your intake form you consent to the collection, use, and disclosure of your information as described here. You may withdraw consent at any time, subject to legal or contractual restrictions, by contacting us at healthylumberjack@gmail.com. You have the right to access, correct, or request deletion of your personal information, and to obtain information about our practices and any service providers.

4. Disclosure to Third Parties

We share personal information only with licensed dietitians providing your care, booking and communication platforms that support our services, insurers or benefits administrators when authorized by you, and as required by law (for example, to respond to a court order or regulatory request). All third parties must meet Canadian privacy and security standards and enter into confidentiality agreements.

5. Data Retention and Storage

Health records are stored securely in Canada with appropriate encryption and access controls. Clinical records are retained for at least ten (10) years after your last interaction, or longer where required by provincial regulation. When records are no longer required, they are securely destroyed or anonymized.

6. Safeguards

We implement administrative, technical, and physical safeguards, including role-based access, audit logs, encrypted transmission and storage, and regular security reviews. Staff and contractors receive privacy training and are contractually bound to maintain confidentiality.

7. Cross-Border Transfers

If any service provider stores or processes data outside Canada, we ensure comparable protections are in place through contractual commitments and security due diligence. Your information remains governed by Canadian privacy law even when processed abroad.

8. Updates to This Policy

We may update this policy to reflect changes in legislation, standards of practice, or our services. Material changes will be communicated through our website or directly before they take effect. Continued use of our services after an update constitutes acceptance of the revised policy.
